Why Do I Do So Poorly On Tests?

Why do I do so poorly on tests, especially finals? I study hard, I don't get nervous, and I don't rush on the tests. I take the time to read each question carefully. I think about the answer I've given and I come out thinking I've done pretty well. Why then do I get C's and D's and lately F's on my tests? I can't get these kind of grades, especially in two years when I'll be in college. Can anyone help me to understand why I do so poorly? Does anyone have any advice for me? Any help is GREATLY appreciated.
