One of my intubated patients (dominant-hemisphere meningioma that beld) had been "chugging" really bad on the vent. I'd been suctioning for scant sputum. The nurse giving me shift change repor said the intensivist said his PSV was set at too low a rate. He was in a hurry to go, so I didn't have chance to ask why this helped. Before *his* shift, the previous RN had given him 800 mcg fentanyl, 15 mg Versed, 200 mg of hydralazine, and 60 mg labetalol for high SBP (>180) and high ICP (in the high 20s to low 30s (mmHg)). After the PSV was raised from +6 to +10 (cm H20), the patient's BP was under control and he was settled, needing only 200 mcg fentanyl and 20 mg hydralazine on the shift befor mine, and absolutely nothing on my shift. What is the rationale for increasing the pressure support? Also, what is "autopeeping?" -- his PEEP was raised to +15 to prevent autopeep.

I have heard differing answers to this by two reliable (But clearly not) sources - The Guardian & The Times.

1, It is after the new moon at the time of (or just before) the summer solstace - The flying Ants are reacting to a tidal pressure change.
2, It is a reaction to the sound of rainfall on the surface of the ground - They happen to be breading around the summertime and therefore the first of the summer rains releases them.

I find answer 1 to be much more believable than 2, Mainly because rainfall is not uniform across the country so it is much more likely that the reaction of the Ants is of a tidal origin. However I have never travelled the length of the country on the day that these Flying ants appear so it is possible (However I feel unlikely) that localised rain sets of colonies near to me.

All I know is that Hepatitis virus A only causes acute hepatitis and resolve by itself after some weeks. I never heard that it causes chronic hepatitis. What I know of is that HepB and HepC are the one that can cause chronic hepatitis...Am I wrong?

Humans need to eat to survive. We need to reproduce to survive. When we have sex it's enjoyable... i.e an orgasm. But when we swallow food our body doesn't experience the same euphoric high. Is this a flaw of nature... or did our God do this to us. We must eat to survive, so why is it not as enjoyable as sex...?
