My doctor prescribed me ritalin, and I went to the pharmacy to get it filled but my insurance denied it saying i was too old and said i had to get a prior authorization form completed by my doctor. So i paid for the meds anyways assuming the doc would fill out the form. Anyways the meds made me not sleep for 8 days then I had to schedule another doc appointment. She then gave me Strattera samples and told me to stay off the ritalin. I told her about the prior authorization form and she said have the form faxed to her and she would take care of it. Now her secretary is telling me that the doc wont fill out the form because ive already been given Strattera! What are my rights here? Does my doctor have to fill out the form? I want to be reimbursed for the Ritalin.

the summer is good to them, but in the winter they spend all day and night swimming from hut to hut in total darkness, fighting with other rats, trying to avoid mink, fox, coyote and traps. swimming 10 yards to find the hut frozen shut and repeating this over and over if you don't drown in the process.... yuck

Hi,

I was wondering how I can get the mean from a table that has intervals (ie: 1-5,
6-10,11-15 etc.). I think I know but I produce an unexpected result. The way I
know it to multiply the Frequency column by the mid point (in 1-5 the midpoint
would be 3), totalling the Frequency x The Midpoint and dividing it by the total
Frequency. Is this correct?
