I'm sure most of you reading this have heard that song by Sheryl Crow.

However, I've heard that term before, and I was wondering where it came from, and what it's real significance is. I know in many cases, these phrases are adopted from something else and given a new meaning over time.

I'm thinking something surgery related? As in the first cut needing to be the deepest, and the psychological depth to it (for the surgeon).

Some people assume that the phrase pertains to a fish called "fluke" but my microbiology teacher said that the phrase has something to do with the parasitic helminth called "flukes" (scientific name Trematoda).
