Some examples from my own brain:

Sinks designed to spray water toward the basin and out onto you.
The same sink placed at the crotch level of your average male.
Parking lots designed where the exits are one large blind spot.
Cell phones that almost guarantee you to hang up on someone when you grab them to answer them.
Automatic towel dispensers. Do these things EVER work?
