I understand what age group is appropriate for which rating but how do the they choose which movies are "R" and which are "PG-13" is it based on content?

I'm engaged and have a child with a man who is 20 years older than me. Commonsense tells me he wont live to a ripe old age, he smokes, is overweight, and has a horrible diet. I want to know how long he will live, I know it sounds like a horrible question, but I worry and think about it so often I have to ask. Can anyone help me?
