Any good doctor will know. Smokers have a bad smell they aren't aware of, but others' can't help but notice. The fingers they hold the cigarette or cigar with will be permanently stained by the nicotine, and the "dry" cough smokers have is also a give away because a non-smoker will may have a temporary cough from a cold, but the smoker's cough won't go away. Listening to your lungs will give you away, too.
