what are the best meds for people with diabetic neuropathy?
my dad suffers greatly from it and we would like to know of some new meds to consider. please share your stories and side effects.

My husband has diabetic neuropathy. Here is what his Endocrinologist recommended : Neurontin 300 mg 3x daily. He has been on this medication for years. He has not had any bad side effects. It helped more when he first started taking it. As his diabetes worsened, they are still effective, but not as much as before. He also put him on fish oil soft gel 1000mg, 2x daily and Alpha Lipoic Acid 200 mg x3 daily.These have been shown to help relieve pain associated with neuropathy. You can find these with the vitiamins at Wal-mart, or any drug store.

Another medication for neuropathy is Cymbalta. My husband has not tried this. You might ask your dad's doctor if this would be a good choice for your dad and the side effects.

Here are some suggestions that might help your dad:

1. Keeping his blood sugar under control. High glucose or up and down levels can cause his neuropathy to worsen.

2. Exercise. I know he probably doesn't want to hear that, but exercise increases circulation and stimulates the growth of new vessels which can help slow down the progression of neuropathy. It can also be a distraction from the nerve pain in his feet.

3. Try Capsaicin ( the active ingredient in red pepper power). When applyed to the feet it acts as a counter- irritant and can help decrease neuropathy pain. You can find this at your local drug store or Wal-mart.

One of the best things is to bring his sugars (if they aren't already) under control. That should be the absolute first thing you do. I would highly suggest a very low carb diet (6 g carb for breakfast, 12g lunch, 12g dinner, with protein/fat to balance it out).

**Just because the pain of diabetic neuropathy may seem to go away, don't be so sure. As the disease progresses it is more likely that the pain is still there but the person can't feel it because of nerve damage**



For painful neuropathy (not just diabetes related) a common first line drug is becoming gabapentin or other related meds. It is has a better safety profile than some tricyclic antidepressants (which are also prescribed for this purpose) but can worsen glycemic control in some diabetics. Some of the other drugs used in non-diabetic neuropathy have not been adequately studied in diabetics and so should be used under strict physican control.

To be fair, the mechanisms of diabetic neuropathy are still relatively poorly understood and many of the medical treatments are to help with pain --- neuropathy does seem to be progressive in most cases. Strict blood sugar control can help to reverse the process if it is initiated soon enough.

It is important that your family member is seen by a neurologist or a diabetes specialist extremely familiar with neuropathy (you'd think they all would be, but that has not been my experience).

Type 2 diabetes, usually referred to as adult onset, is when your body produces insulin but can not use it properly. This type can be treated successfully with the right diet and exercise in most circumstances. If blood sugars are not kept under control at some point insulin will probably be integrated / oral medications are also available.
People with diabetes can live long, happy lives but must be diligent in their care and make sure you see your endocrinologist on a regular basis.

As with any medicine or diet change, you should discuss it with your doctor. Fruits, both fresh and dried, have a natural sugar in them that will raise blood sugar levels, so be careful about eating too much. Not sure about the nuts. Moderation is always the key. I've been diabetic for 18 years and just recently changed insulin types. I love it because it gives me more freedom in when and what I eat.
