Well eventually he will get worse, start to lose his extremities and then start to lose his sight. So you might want to tell him that unless he desires being an amputee who is blind, he should get back with the program!

A lot of things can happen, there is no way to be sure of it all but one thing is certain: His serum blood sugar will rise.
You say he was being treated with tablets, which indicates type II diabetes which can be mostly controlled with a very strict diet, but without the pills it could be near impossible. The cholesterol and blood pressure meds are probably preventative and may not be essential. I can't say for sure with so little information.
He can get ill from refusing his medication, but what is of utmost importance right now is to know WHY he has stopped taking them. You need to find out if he has a clear head about this, if he hasn't or if his reasons are because the treatment is giving him side effects that are debilitating it could be a symptom of a different illness and not just him being stubborn.
Talk to him about his reasons for refusing the treatment and ask him to talk to a health professional about those reasons. First be sure what's behind it, then decide the next step.

I am writing to tell you what an incredible impact these methods had on my life! I have had type 2 diabetes for 27 years. For me, the worst part of this horrible disease is the severe pain I constantly get in my feet. The pain is so bad that I avoid standing and walking as much as possible. I've got to tell you that within the first month, my feet stopped hurting altogether and I can now walk totally pain free.
Believe it or not, I even danced at my niece's wedding last month, something I have not done in a many years. I've been following the book for six months now and my blood sugar is well within normal range. I feel great!
I recommend you use the Type 2 Diabetes Destroyer to naturally reverse your diabetes.

First of all, is the medicine making him feel badly, or is it plain stubbornness? If the medicine is at fault, there are others that can help. I know that diabetic meds often cause stomach distress, which is why I had to stop mine. However, he should know that he is making the decision to have high blood sugar, which will make him feel terrible, but also cause him to have problems in the near future. I am under 45, have had uncontrolled blood sugar for at least 4 years, and I just had a heart attack. The cause? Microvascular disease which is usually caused by diabetes destroying the tiny blood vessels. He should know that even if he doesn't have health problems overnight, there is no doubt that they are approaching, and perhaps sooner than he thinks.

Class I or Type I diabetic would go into a coma and eventually die.
Class II or Type II diabetic could do well if he eats right, exercises and takes really good care of his body. This may be what your friend is trying to do. Watch out after him. Check blood sugar and check blood pressure.

The possibility of a heart attack can not be ruled out. He should take aspirin daily as preventive measure in consultation with your doctor. He should check blood glucose and blood pressure weekly if not daily. As regards diabetic and hypertensive patients, the administration of anti-lipid drugs is a must even if the cholesterol level is normal.
