Open heart surgery (OHS) is a general term for different kinds of surgeries performed on the heart. It usually involves putting the patient on a heart-lung machine and stopping the heart. It is most often done for CABG procedures (Coronary Artery Bypass Grafts) for people with blocked arteries to the heart. It is also used for valve surgery like Mitral Valve Replacement or Aortic Valve Replacement. These are just a few examples. There are a lot of congenital defects that are also done during an open heart procedure.

It could be many things. The technology has improved a lot, if you need to replace a valve or to install a pace maker, both can be a open heart, but the scar will be very small, around 2 inches.
