Heterosexual couples are more likely to spread HIV from infected semen entering the woman's bloodstream through vaginal tears.
Conceivably, an infected man could transfer HIV to a woman if she performed oral sex and an open sore in her mouth came in direct and prolonged contact with his ejaculate. He could also bleed from small nicks on his penis and transfer HIV that way. It would be a very remote possibility, but the mechanism does exist. An female-to-male transfer would be even more unlikely, but not completely impossible. Her infected blood could enter his penis through tears in the skin.
All this to say this-- use a condom or dental dam.

The chances of contracting HIV from oral sex is very low, around 2.8 %. This does not mean that you should be careless. There is still herpes, and other STD's to consider. If you are unsure about how "clean" your partner is. Give head with a condom or dental dam.

Yes, you can easily spread HIV/AIDS or other STD's by oral sex. The person on the receiving end if the one most like to get the disease no matter if it is oral sex or penis/vagina or penis/ rectal sex. There is no such thing as "safe sex", but unprotected sex is the most dangerous.
