And name it after the person who died?

NO! It's a sweet thought, but a really, really bad idea. One, they may not WANT a puppy. Puppies are a lot of work, and a bereaved person is probably just not up to taking care of a puppy, especially if the deceased has been ill for a while and they were taking care of them.

Two, naming a puppy should be the privilege of the person who owns it. Giving someone a puppy and saying, "Here's Malcolm!" is waaay creepy -- like you think he's been reincarnated!

No. One, it is rarely a good idea to give an animal as a gift. Two, if you name it after the person who passed away, the bereaved person may not take that well. They may see it as an intentional insult.

Flowers, sympathy cards, donations to a charity in the name of the deceased... these are generally safe ideas.

Naming it after the person who died, no.

Giving a puppy depends on the individual. Some would like it a lot, and some wouldn't. Maybe bringing a puppy over to visit for a while would be better--cheerful presence of an endearing animal without the responsibility. After a few visits ask the person if he/she would like to have the pup.

After time has passed, a pet is a wonderful addition. A dog gives much love. But, you'd have to make sure it's okay with the recipient first.

Naming it after the deceased is very funny in my book, but I do not know if anyone else would get it.

No, it isn't. You may tell the person if he or she would like a puppy, you would be pleased to get it for him or her. If that occurs, you should give the puppy to the person unnamed.

No, for two reasons
1: it means they have to be responsible for it
2: It minimizes their grief by unintentionally implying that their loved one can be replaced in their hearts by a pet.
