You must put in:
How much you charge.
If after a certain time you charge more per hour.
If there is more than one child how much extra you charge.
Who is to take you home after babysitting is over.
If you must do other chores beside
watching and feeding the child how much more you charge.
If you get hurt while babysitting or if the child hurts you, who pays the doctor or hospital bill.
Are you allowed to eat what is in the house to eat if you are hungry.
Must have phone numbers of where the
parents are going, their cell numbers, and numbers to close relatives of the children.
Parents must make a list of activities children are allowed and not allowed to do
and list of foods allowed and not allowed to eat etc.
Will freinds be allowed over when you are babysitting? Put that in also.
What should you do if the child doesn`t
listen to you? Put something about that in
also.
That is all I can think of now. Hope it helps.

Well if your the provider, you need to put payed days off, if they are responsible for paying even when child does not come, holidays schedules, when payment is due and late charges, you should add that if they are a certain amount of days late that they can not bring the child. Anything you can think of that would be an issue, people will take advantage of any little thing.
