What your neighbor is doing is considered as neglect in a court of law. It doesn't matter how intelligent and kind the dog is, that toddler is still considered as being left alone, the same as if the dog were not there at all. How would that dog know if that child were to get into something poison for example? Can the dog read or even know what the skull and crossbones mean? Something needs to be done and you should call the Department of Social Services Child Protective Division to make the report. You can do so anonymously if you like, but it doesn't matter. They cannot release the name of the person who calls them with a report. If your neighbor knows you are the only one with this information, it will be easy for her to figure out that you made the call, but still, that child is in danger with her leaving it this way and it is a very stupid and irresponsible thing for her to do. Something is wrong with her for leaving the care of her child to the dog and thinking this is all right! You should make that call right away. Doing so may save this kids life!

No I would not let my child run around the streets naked. The only time my son really didn't wear clothes was when he was potty training, if he had nappy rash or just after a shower... and even then I would always have at least a shirt on him. Some nakedness is ok for toddlers (this girl isn't really a toddler anymore if she is 4) as long as it is in the privacy of your own home or yard. It's not that difficult to stick a shirt or pair of pants on your kid. She is at risk of infections, sunburn, hypothermia, frost bite (depending on the weather), injury from broken glass etc and creepy people. That is very negligent of the parents and I would consider reporting them for negligence from what I have heard.

Call CPS. Whether the dog is safe around the child or not it is not legal to leave a toddler home alone. IF something happened she could be charged with child endangerment and if the child gets hurt by another means the dog can't call 911.

Even a "very intelligent and kind" dog cannot cook a meal, put a kid to bed, or perform CPR. Children need to be looked after by qualified adults... or at the very least, a qualified teenager who knows how to babysit.

Call Child Protective Services, and/or the police. And do it every single time you see the mom leave without a caregiver arriving.
That dog probably can't change a diaper, prepare a meal, or deal with a smoke detector. He might be adequate in the event of an intruder, but there's so much more that could happen.
That woman is an idiot.

Do you mean just outside in the yard, or really leaving the baby in the dog's care?
The dog would of course probably protect the baby from kidnappers, and raise an alarm if something bad happened; but if she's leaving home altogether without a human babysitter, it needs to be reported.

And another big fat WOW!!! Call CPS, that is irresponsible and dangerous. I don't care how intelligent or sweet a dog is, a dog is a dog, any dog can bite and if the child does something to the dog, IT WILL BITE... how unfortunate that this woman is so deluded and has a child...

If you can't talk to her... you might want to consider letting social services know. Dogs have been known to go to great lengths and perform feats of incredible intelligence to protect their family, but that doesn't mean it knows what to do if the baby tries to eat something it shouldn't or if the baby climbs up on the counter or something like that.
