The type of surgery you're describing is called maxillofacial surgery, and it is a type of reconstructive surgery dealing specifically with the jaw. First of all, if your underbite is just slight, braces should be able to fix the problem without any surgery at all. However, if the problem is due to an underdeveloped, pushed back kind of lower jaw, maxillofacial surgery might be for you. What they would do is put you under anesthesia and basically break your jaw and move it forward, then re-set it so it heals properly in alignment with your upper jaw. I had an ex-boyfriend who actually underwent the procedure (mostly for cosmetic reasons, I think) and he said it was rather painful and annoying recovery (because they have to wire your jaw shut while the bone heals, so you can't eat anything but liquids), but it may be worth it if you have a real problem. Also, if the reason for needing the surgery is medical, like you can't bite properly, your insurance should cover at least 80% of it, but if it's just cosmetic, it will probably cost you like $10,000. A surgeon wouldn't fix your under bite. An orthodontist would, and they would probably put a retainer in your mouth, or put two retainers in your mouth and attach rubber bands to each retainer. That might cost a bit of money. Not sure how much.

You don't need a surgeon, you need an orthodontist. The cost would depend on the severity and other factors such as insurance.
