Is a handwritten doctor's note considered " medical documentation" ?

Well if taken literally...it is a medical document...or documentation as you put it...

so yes whether hand written or typed...as long as it is signed by the doctor, it is a medical document

It can be, depending on the circumstances.

If you need medical documentation for being sick, then a doctor's note on the office letterhead (with contact number) will suffice.

If you need medical documentaton for something more serious, they'll likely ask for test results, x-rays and other things along with a doctor's note.

It all depends on what you need it to say.

Sometimes, a simple note from your doctor would suffice. Sometimes it requires extensive documentation with folloup history. Sometimes, it needs to follow a certain standard (forms).

Your best bet is to go back to the person/firm that requires the note and ask them exactly what they need/want.

For example, if you signed up for a travel insurance and need to file a claim, you'll need to have your doctor fill out a form that the company provides. It needs to have some history and complete diagnosis, AND the statement that the doctor does not recommend that you'd travel.

Anything written by your doctor is documentation. It also depends on what you need it for. Government gets picky about certain things, but if you doctor wrote it, then have to take it into account. Best of luck.

In most cases yes unless the person requesting the documentation makes it clear on what documentation they want to see.

yes typically but you can ask your doctor to type something up on his letter head so it looks more official, they usually dont mind doing it.
