Pump the cuff up to 200, and listen for the 1st beat record the number and listen for the last one and record the 2nd number.

You can use the cuff, its automated, and it is pretty accurate, I would venture to say even more accurate then manually doing it, due to human error. But if you must do it manually, the first answer is correct. Also, and this is only in the ICU setting, there are patients with A-Lines, which is a catheter that goes in your artery and constantly measures it.
