refers to Jesus- this is bible prohecy predicting that Jesus would be born SPECIFICALLY at Bethlehem ephrathah- when Jesus was born- there were 2 bethlehems at the time- one in the south and one further north- mary and joseph were forced to go south to get registered as decreed by pilate even though they lived in nazareth closer to the "other" bethlehem- so they came down to jerusalem and Jesus was born in Bethlehem ephrathah like was prophesied-
