A 1040 form is an I.R,S, tax form. It is what you use when your yearly taxes are filed. I believe you need a W-2 which is an employees wages record.. On it is what you made, how much taxes were withheld,what city or municipal taxes were withheld, and what state taxes were wit held.. Also your F.I,C.A. ( social security) monies were wit held.. You need to contact whoever wrote your payroll check every week, or the party to which had you fill out a W-4( how many dependents you claimed.. Whoever that is, has an obligation to furnish you with a W-2 which has all that information on it... SOLOMON

A Form 1040 is not provided by the employer. It is your tax return, and you should keep a copy when you file the return. The employer provides a W2 form by January 31 of the year after you worked there. If you worked for Jiffy lube in 2007, they will send you a W2 form. The W2 form shows the wages you earned and the amount of income tax and social security tax deducted. You attach a copy of the form to your 1040. The 1040 form is available from the IRS.

Are you needing the W-2 (wage information) for a previous job for years 2006 and prior, call IRS at 1-800-829-1040 or download Form 4506 or 4506 T for a copy of the w2, Free of charge. If you need the W2 for the year 2007, you must contact the employers main office or call IRS after Jan 31, 2007 and let them know you didn't receive the w2.
A form 1040 is the form you use to file your tax return each year.

I believe you want a W-2, not a 1040.

If it is a 2007 W-2, they are to be mailed to by by 1-31-08. You should contact your 2007 employers with your correct address if you have moved since working for them.

You get a W-2 from an employer, not a 1040. You should get it from them in January if you worked for them this year. If you are looking for info from a previous year, check with your previous employer - if you can't contact them or they don't provide it, you can get a copy from the IRS.

you are looking for a w-2, a form that details what you were paid and what taxes were withheld from your salary, so that you may complete your 1040 form for your federal income taxes. i would call the shop and start there. if they outsourced their payroll, they'll be able to obtain a copy from the service.

You can get your W2 from the owner but they should mail it to you in January.
