You can void the PS3's warranty if you open the PS3 yourself and try to mod or fix the console yourself. Also if you drop it, something on the inside of the PS3 might be knocked around and some parts inside may break off.
But other than that, the PS3's don't become defective because the PS3's hardware failure rate is less than 1%.

Dust also voids your warranty, so as everybody that owns a PS3 knows it attracts dust like a magnet. Sony apparently won't honor a warranty if dust is found to be the culprit in a malfunctioning unit.

USER ERROR: The user tries to MOD it. The user installs an incompatable OS and then doesn't know how to restore the original factory settings and Firmware (which is quit easy)...
