I have had three kittens spayed in the last couple of months. No pain medication for any. Kittens are quite 'durable', and bounce back fairly quickly. I suspect your vet has offered pain medication as an extra cost item, and it really has more benefit for you than for your kitten. If you are really concerned, you can put forth the suggestion of pain medication only if it seems really necessary, and most vets will honor that wish. Ever wonder why a spay can run from $75 to $250? It all has to do with the extras offered, and for some, it is not for the benefit of the kitten.
Your kitten will be a bit under the weather for a day or so after the operation, but that is because it is major surgery, and anesthesia is used. But it has the benefit of slowing them down for the period right after the surgery.
Your vet will probably tell you to keep the kitten from going up stairs, jumping on furniture, etc. Have a good laugh, and realize they self-regulate quite well.

I would go with what the vet recommends. One of my cats had no pain at all after her spay surgery. She came out of the anesthetic fine & the vet didn't even send any pain medicine home with her.
The younger & much smaller cat had a more difficult time of it. The vet said she didn't come out of the anesthetic as fast & she moved very slowly for the first day. I gave her the pain medicine the first night & again the next morning. After about 36 hours, she was fine.

Morphine cannot be given to cats as a painkiller. It causes manic behaviour, which is the last thing you want in a kitten who has just had surgery.
The vet is the best placed person to advise you on safe pain relief for your kitten if necessary.

Niether, a little pain has a purpose, you don't want her exerting herself and breaking the sutures. If you think that is cruel and are wondering if I would do the same, the answer is yes. I have never given my pets pain relief after surgery, and I myself did not take pain relief after surgery, it hurts a bit but it helps maintain limitations.
