I assure you the answer is "D." An indictment is a formal accusation.
The individual with the law degree must've missed classes the day "indictments" were discussed and failed to learn that most cases start in the district court with a preliminary hearing. The District Court will remand a felony case to the Grand Jury where the Grand Jury makes the "formal accusation" know as a "true bill."
Once a case is true billed...the felony case is handled by the Circuit Court instead of the District Court!

Whoever wrote this question has not given you sufficient knowledge to give a correct answer, since no jurisdiction is identified. In California, the document which follows a preliminary hearing is an information. An indictment is returned only by a grand jury, and no preliminary hearing is conducted. I believe this is true in many other jurisdictions as well. Do you have any idea what jurisdiction is intended?

I believe you are seeking for an "arraignment" - none of which you leave as an answer is correct - an indictment is handed down by a grand jury within a week of the arrest in which the defendant has already been arraigned - formerly charged with a crime, he/she are read the formal charges and a court date is set - if a felony in NYS a 180.80 day will reveal a Grand Jury decision whether or not substantial evidence exists for a felony charge.
