About every 2 months. In between, I drop it in the silverware holder of the dish washer at least every 2-3 days.
I don't know if you have ever thought about it but if you have a cold that you can't seem to get rid of, it is often because you keep reinfecting yourself every time you use your toothbrush. So run it through the dishwasher every day if you have a cold or if someone else in the family has one (those viruses migrate from one brush to the next when they are all sitting in the cup beside the sink.) When the cold is over throw the old one away and get a new one. Yes I go through a lot of toothbrushes but I rarely catch a cold, sore throat, or stomach virus.

A toothbrush should be changed out as often as necessary, which is usually once every 2-3 months. If you find that the bristles are bent or out of shape before that time then it could be that you are brushing too hard, which is not good for the tissue or the teeth. Always make sure your toothbrush is clean and that the bristles are in good shape, if not, then change heads or buy a new toothbrush.
