This could very well be malignant blood pressure, it is a medical emergency that needs attention immediately, especially if accompanied by a severe headache. Call an ambulance and calm him down if a heart attack is suspected.

With the symptoms you've enumerated, they are more like hypertensive symptoms than heart attack.
Heart attack manifests myriads of ways, but most often, it presents as chest pain.
Excruciating chest pain with the thought he's going to die, is probably the most plausible layman's gauge if one is having a heart attack.
What to do? Call the 911, nearest hospital, the EMG, etc.
If the patient is unconscious, check for pulse. If you don't feel one, give one precardial thump (job the chest where the heart is) and do CPR.

Calling 911 is not a bad idea. Definitely talk to a doctor. Get some 81 mg aspirin. Have them chew one right away.
Then have them lie down, and try to be calm. Lift the feet a little.

If possible give the person 1 aspirin 81mg, & call the paramedics right away. He went to the Dr & what medication did he get? As far as I know, he was supposed to get @ least aspirin, and propanolol(indiral) to protect his heart. Good luck.

For saftey reasons he should go back to the doctor.
Bayer has been proven to stop/prevent heartattacks. Keep some with him at all times. If he starts to have one he should take the Bayer. It may save his life.
