Some children are born with it and some develop it over time. It doesn't necessarily only happen because of smoking, although that is one cause. Allergies, and such can also cause the development.
Now, there is also a misconception that even some doctors believe. That people outgrow asthma. This is a chronic disease and you don't outgrow it, but you can control it. There are many people who "seem" to outgrow asthma. In fact, their lungs gained strength to self control the episodes or attacks. But later on in life, or in certain conditions can cause it to rear it's ugly head.

There may be a predisposition to asthma, but newborns don't ever have asthma. Once not that long ago, it was unusual to see a kid under age two with the disease, though over the years asthma has become more prevalent and more severe and occurs at younger ages than in times past. Those are, of course, generalizations, and don't apply to any individual, but it's nonetheless true.

Asthma is classified as either acute (a single episode) or chronic (long-term condition). Asthma is also classified as either extrinsic or intrinsic.
Extrinsic asthma involves acute episodes triggered by some type of hypersensitivity (allergy) to an inhaled agent. There is also usually a family history of other allergic conditions like hay fever or eczema, and the onset of this type of asthma occurs in children. Some people actually grow out of it when they reach adolescence.
Intrinsic asthma usually occurs during adulthood. The things that trigger these types of attacks are respiratory infections, exposure to cold, exercise, drugs (like aspirin), stress and cigarettes. A lot of people unfortunately have a combination of BOTH types of asthma.
So, in response to your question, some people can be born more susceptible to asthma attacks (due to family history and/or environment, etc.) but they don't necessarily HAVE asthma, they're just more prone to get it. As a child, it has more to do with allergies and genetics, and as an adult it deals more with environment, allergies, and overall health. :)

This coming from an Asthma patient, I was diagnosed with Asthma when i was 9. My dad smoked a LOT around me as a child. So, with me, it's second-hand smoke. People are born with Asthma though. If you have Asthma, you can develop long-term effects of Obstructive Sleep Apnea (OSA), which I was recently diagnosed with. If this is regarding your child, or someone you know, bring the child to a doctor, and they have simple breathing tools they use to diagnose it. If your child has Asthma and it's not severe, likely, he/she would develop OSA.

Both. Some children are born with asthma but usually grow out of it. Some children develop asthma later in life with no specific cause. Shortness of breath (SOB), wheezing, or coughing after strenuous activity or allergies. Sometimes there is chest pain or tightness.

A doctor told me that one of my children was born with asthma, while another one developed it when he was 3.
Some of the signs I noticed with my little boy was a persistent cough, wheezing, loss of breath from very simple task. I also noticed for both my children with asthma that the winter time is much worse for their systems. I don't know if that's the case with all asthmatics, but it certainly is with my kids.
