My relative was born with a hole int heir heart that the doctors never knew about so eventually the hole grew bigger.

But they said that the exact cause of death was a heart attack and a stroke in their sleep. I just don't get how it can be both? Can someone explain this to me? Thanks.

The precise cause of death is possible after autopsy, the rest are hypothetical.

A blockage in an artery (blood clot, e.g.) can cause a heart attack or stroke -- or both at roughly the same time. They probably said it was both heart attack and stroke that "caused" the death because they knew both had occurred, relatively simulataneously, and they couldn't be 100% positive which one was actually responsible for your relative's death. But it's really not important which one specifically caused their death -- your relative might've been able to survive one or the other alone, but in combination, it was too much for their system to handle. I'm very sorry for your loss.

A stroke is the result of lack of oxygen to the brain. A heart attack which can have many symptoms (effects) can cause a stroke.
