Blurriness is caused by the lens. Damage to the optic nerve changes the way you perceive the visual image. Typically, nerve damage changes things like resolution, field of view, color perception, and contrast. Optic nerve damage is more like changing the exposure on a photo, as opposed to a photo that is out of focus.

Yup. Mostly.
Since this is in the "Diabetes" section: vision impairment is a common symptom of untreated diabetes.

It won't make you near-sighted, though. Near-sightedness or far-sightedness are a result of the eyeball being the wrong shape.
