It can, but it isn't a very good choice. The vast majority of upper respiratory infections, whether viral or bacterial, are affected very little by antibiotics, and it's nearly impossible to predict in advance the few that will require any antibiotic. In those that do, cephalexin works well against streptococcus species but not so well against any of the other major pathogens.
Thirty years ago it worked quite well for pneumonia, urinary infections, skin infections, and a variety of others. Drug resistance patterns have chipped away at its utility, and now even simple skin infections once treated using the drug with confidence, especially when the presumptive organism was staphylococcus aureus, now require the addition of a second drug to cover for MRSA.

Not the best antibiotic; keflex ( afirst generation cephalosporin type of antibiotic) is used more for skin & wound infections. It has greater gram positive coverage ( a class of bacteria which includes strep & staph) than gram negative coverage (the second type of bacteria). If the respiratory infection was caused by a gram negative or even an atypical bacteria keflex would pretty much be useless. Many strep bacteria are resistent to keflex as well. So, bottom line, if someone has a respiratory infection, I would not give keflex.
