I think God hears both.

I prefer to pray quietly, alone, and usually not out loud.

He can hear thoughts and the heart or words...and I'd suspect even in this life--to a lesser degree-- many angels / demons can do the same. In short, there's nothing God created that He isn't aware of.

btw-
Thinking thoughts and normal thinking in your head is NOT a mental disorder. Having belief in the Christian God (or any religion) is NOT a mental disorder either.

Prayer would not artwork, that's why he would not answer your prayers. Prayer is "replied" only as probable as could be expected via random possibility on my own. This has been shown scientifically, by means of dozens of experiments.

God knows what you want before you ASK it. So if he know what you want before hand it would be effectual to ask it. But just because we ask doesn't mean we will receive it, it may not be in our best interest sometimes! Those would be considered our un-answered prayers as we call them. if god knows, when you're not praying, then he should hear us when we pray in our mind or in our heart or when we say it out loud
