It's probably a precautionary deal on the part of the manufacturer fearing liability issues. Poorly controlled diabetics tend to be at risk for circulation problems. Maybe the massager can interfere with circulation.

Well controlled diabetics should have no problem with it, but you know how it goes... CYA

a very common effect of diabetes is know as diabetic neuropathy which causes numbness of the extremities of areas such as the back. to prevent getting injured and not noticing at that particular moment these kind of devices as well as many other over the counter massaging machines are not reccomended for people with diabetes.

I would think because of the poor circulation, there tends to be more pooled blood and clots in the body. Using a massager may loosen up the clots and send them to the heart or lungs, probably the two places you most don't want clots to be.

Diabetics have poor circulation and poor healing, so it would not be a good idea to use machines or other similiar things.
