well that's a pretty broad question, but basically when the heart is "overworked", it can look enlarged on xray. the patient could have congestive heart failure or coronary atery disease or both. some symptoms would be shortness of breath, perhaps chest pain {but not always}, tachycardia (faster than normal pulse) and tire easily. you may also see discoloration of the nailbeds of their fingers if it is gone long without treatment. after a while, the body starts to compensate for all of these so you may not see extremes of the above, however, this person should be under the care of a good internist.
