Sorry, no such thing.
The problem is that people differ. by height, weight, activity and metabolic rate
So using any "calculator" while in insulin therapy just might get you killed (too low glucose, once). Or blind (too high glucose, too often)
Best choice is to start with ultra rapid acting insulin and test blood sugar, and make notes.
OK, it means a dozen injections in a day, for maybe a month or so- but then you know how you react to therapy and know how long you can dance at a disco or play basketball before you need a snack.
Also works the other way- how much insulin to take after that birthday cake...
The biggest danger is too much slow acting insulin (8-12 hours before it starts acting) and forgetting to eat. Keep some glucose tablets, or a chocolate bar, or a bottle of standard (NOT diet) Coke - or all three - handy

Dosing varies people to person depending on the amount of insulin produced by pancreas.You need to be monitored initially by MD, and get initial serum testing. Dont mess around hypoglycemia can be deadly, blood glucose < 60 is critcal.

Then you should consult a qualified endocrinologist who specializes in treating insulin dependent diabetics.
The reason is, there's no "simple" table. There are too many things to consider... like your weight, how much and what types of foods you're eating, etc.

you need to get your sliding scale from your endocrinologist. They are the only ones whom can do this for you. It is based on your journaled blood sugars. Don't try to do it yourself. There are too many different doses for different days, sick time, etc.
