A doctor can see evidence of "sexual trauma" when s/he does a pelvic exam. So if you have a lot of rough sex, your vulva and vagina can appear swollen, red, scratched up. And in the vagina, if the lining is red, swollen or bleeding, those can be interpreted as signs of sexual trauma as well (depending on how traumatic it was, I suppose). A simple swab of the vagina can also find traces of sperm if you did not use condoms during sex recently.
btw, the "lips" are called labia. The labia majora are on the outside (the part where hair grows) and the labia minora are tucked underneath and have no hair.
