I guess it is arrhythmia....Tachycardia, it depends on his "T" wave analysis. It can be Ventricular Tachycardia.

Why is he taking the testosterone? Is he taking any other medication? It would be unusual for a 60 year old male to not be on something else. Aspirin?

Testosterone will increase energy, hair growth. It can also increase aggression. So, perhaps the testosterone is affecting him mentally, making him aggravated. Anything is possible without more information.

Sounds to me like he needs to take up yoga or tai-chi. Water aerobics or something. The 30 lbs is not a huge concern.... but it never hurts to get a little low impact exercise. At 60, it's not so much the cardio as it is the movement. You don't have to get your heart rate going crazy, you just need to keep the blood flowing.

I would have them do an ultrasound of his chest. If his heart is under too much stress, he needs to be on a different med.

I'd look at the medication as the culprit. 30 lb overweight should not be the culprit... 80 lbs, yes, but not 30. I took a partcular blood mediaction for two years with no problem. Then all of a sudden, I had swelling. I never thought it was the BP meds, but it was. Meds you've been taking with no problem can start to act negatively.

Stop taking testosterone and get yourself on a diet ASAFP, meaning right now, before you have a massive heart attack.
