It means when one person tells another person something and then another person tells somebody else. So the information is spread by "word of mouth." Like if there is a a band or doctor in your town who is really good and they don't really advertise, but everybody knows how awesome they are, their reputation gets spread by "word of mouth." Another example is if a girl is a really good kisser and she is known to flirt with all the boys and hooks up with them, her reputation and skills are spread by "word of mouth."
