The hernia needs to be reduced. If it will not reduce onits own, he needs to be seen now. A significant delay in reducing a hernia can result in necrosis (death) of the trapped intestinal tissue. One treatment, which has been used with some success, is to elevate the foot of the bed. DO not bend him at the waist as this increases the pressure on the hernia and makes it worse. Put blocks or something under the foot of the bed to elevate it so gravity can help the hernia fall back into place. IF this doesn't work in about 20 minutes, take him to the ER or call 911.

If the hernia is bulging like this you should go to the E.R. It could be strangulated and it's a dangerous complication of a hernia. The only pain control I can see at home is Tylenol and a heating pad, but I prefer he go to E.R.

As much as possible don't make him stand upright for the mean time but he still has to go to the ER the soonest possible time.

This won't heal be itself.

Call an ambulance and let them transport him to the emergency room. That happened to my mom, recently, and they had to perform emergency surgery on her.
