No ! You wont be in trouble. that's what insurance is for. You pay so that if you need to use it you can! You can drop a policy anytime you want. the only thing that if you signed an annual contract or semi-annual contract they could report you to the credit bureau if you don't pay out the contract term you signed on for. But they can't ask you to pay their part of the bill.

No, you won't be in trouble. But any services rendered AFTER the policy is cancelled - which could be as soon as the day after you quit - aren't covered. Including followup visits, or complications related to the surgery.

Most insurance companies require preauthorization for this type of surgery.
If they preauthorize you AND YOU ARE STILL EMPLOYED AND COVERED BY THE INSURANCE WHEN THE SURGERY IS DONE, they can't penalize you if you return to work afterwards and quit shortly thereafter. You are not in bondage to your job, after all. What matters is the surgery date and your coverage dates. That's all.

There won't be a problem. They cannot ask you to repay it.
Just a word of caution. Wait until AFTER the surgery and you're making a smooth recovery to hand in your notice.
