For me it was in a tournament.
I was fighting a TKD person who of course loves to kick and he kicked so high that his Gi pants wripped open from crotch to foot and were just hanging when he came back down from the kick. He quickly noticed and stopped and the judges stopped and I was too busy laughing to even think about trying to score a point. LUCKY for us all he had underwear on!!!

I once watched a Kickboxing match back in college where a pugnacious older fighter with a pot belly was pretty game and went toe to toe with a younger fighter who was in better shape, they went at it with gusto until the bell rang to end the round, the young fighter turn around to head to his corner but the older guy rushed after him and threw an awkward flying double kick, luckily the younger guy was able to dodge at the last moment after catching the movement from the corner of his eye and the older guy landed awkwardly spraining an ankle. He had to be helped back to his corner by the surprised referee and subsequently lost the match because he couldn't continue due to self inflicted injury : P

I'd go with Matt Lindland or Grey Maynard knocking themselves out during a fight. Lindland on a slam, Maynard on a slam or shot.
Or maybe Pete Sell getting KOd against Scott Smith after hitting Smith hard enough to win the fight

The verry first class/lesson I attended in Karate, the exercise was to, with controll, touch the opponent on the face with your elbow.
First class, not enough controll, so I followed through and gave a 40 year old women a bloody nose and a black eye... she was a blue belt though and should have been able to handle it!

This wasn't me, but I can image it was very embarrassing and painful. I was watching two MA fighters go at it, both looked to be a blend of grappling and kickboxing styles. they would constantly kick each-others legs. But one time they clashed shins and ones leg broke. Seriously it was at a ninety degree angle the wrong way in the middle of his shin. He went to place that leg down not initially noticing it was broken until he obviously lost footing and fell backwards on the mat. Noticing what had happened he began to cry out in pain. The other warrior in all of his infinite wisdom hovered over him and a very aggressive posture and began yelling at him about what a bad a** he was. It was very nice of him to do that.
I image even after the shin healed, and he was able to cope with the fact he may never fight again, that he was still very embarrassed. But then again I can only image.

I remember one time in 5th grade ... it was winter and I had one of those over the head starter jackets with sports teams on them. So I go to take my coat off and hang it on my hook and I actually took my shirt off with it! At that time I didn't have much of anything for boobs so I'm sure it could have been alot worse. Nobody saw me... at least not that I know of. Still it was super embarassing and I still remember it till this day.

I've split my pants so many times wrestling I don;t even care anymore :)
Probably the worst I have seen is on youtube the big heavyweight guy doing jujitsu and shits his pants through his gi. And does not even realise, just keeps wrestling with a stain. :D

loving the possesed canine. the funniest element i observed on youtube became something that pertains to my city. 2 toddlers waiting on the kebab van exterior the pub (or oach coach in case you will), pulling out lightsabers from nowhere, and having a good previous formed celebrity wars conflict.

I was really drunk watching UFC 55. I spilled nacho's all over my friends couch. I took a pee and got some dribble on my shorts. plus the card that not was very embarrising.
