too many people nowadays have forgotten what the proper exchange is.

the only answer to "How do you do?" is exactly: "How do you do?"

it's somewhere between being rhetorical and being a real question. you can consider it a glorified "hello", if you want. it was the proper way to greet strangers no matter what social level you came from, and this was what i learned when i was growing up in New York City during the 1960's, so it is not supposed to have any sense of pretense at all.

(if you want any evidence of this just watch the old "Honeymooners" tv show from the 1950's about a working-class bus driver from Brooklyn, and see how they greeted people they had just met...)

(and what we've developed in the meantime unnecessarily complicates the entire protocol of meeting someone new for the sake of trying to sound "smart" or "civilized". plain and simple is still the best, so i still say "How do you do?")

In America, or in English people generally do not say "How do you do?" anymore.
Here are similar questions like "How do you do" and appropriate answers:

Q: How are you?
A: Fine, thanks, you?

Q: Hey, what's up?
A: Nothing much, you?

If you did want to be that formal though, here's what you would reply:

Q: How do you do?
A: I'm fine, how about yourself?

Good luck! I hope english comes quick and easy for you! :)

In the unlikely event that someone says "how do you do" the correct reply is "how do you do". I can't think of anyone who really says that though. It's said only when you first meet the person.

depends on how formal you want to be. The first answer is formal; the second informal.

"How do you do?" is the correct response when you are introduced to someone.

The proper response to to "How are you?" is
"I am very well, thank you. How are you?"
or
"Very well, thank you. And you?"
