"To Whom It May Concern:" or "Dear Sir or Madam:" are the forms MOST likely to conform to present day usage.

With regards to your question about LEAST likely to conform, if you don't know the name of the person who will be reading it, then how can B and D be choices? Those are names. Your question makes no sense with the answer choices you've given.

"C" is correct, if you do not know what the person's name is. Write "Dear SIr or Madame." That sounds a little weird in this modern day, but "To whom it may concern" is considered to be impolite--according to one of my professors.
