i meditate (deep breeth) for like few minutes to release access stress and thoughts........anymore ideas to help keep focused?

For me I keep in mind that it may be my last prayer ever, therefore I pray to the best of my ability, it really helps!!

Yes, I think you should try and pray alone or with other people who are praying. Try not to pray in places that can distract you. You need to be able to condition yourself to pray..When you know it is about time to pray you can try to read the Quran a few minutes before you pray. You can keep trying to do meditation to clear your thoughts. You can try praying in the Masjid. You can make sure your sleeping well at night and reading the Quran before bed.
If you find something may be triggering your thoughts you can attempt to eliminate doing those things..

There are hadiths where Prophet Mohammed(PBUH) was being distracted by the designs on the shirt he had, so he said to wear clothes that do not distract the eyes and thoughts.. Keep your head clear just for the sake of Allah(SWT)

These are the points u must remember for good concentration in namaz...

(1) After u finish ur wudu, pray namaz immediately. don't delay it. This means, start namaz after fresh wudu.

(2) Don't think of any incomplete work which u left before going for namaz. keep out of reach of ur mind.

(3) In the namaz assume that u are standing before Allah & u are watching Allah. If u can't assume this, then atleast assume that Allah is watching u. Keep this always in ur mind during Namaz.

(4) During Namaz, ur ears must listen what u are reciting. This is Sunnah of Prophet(pbuh). But this doesn't mean that u must recite loudly.

I concentrate really hard on that feeling of submission.....especially when i go into sujood position, i allow myself to feel the worship i am offering to God...and with that feeling in me, all other thoughts are blocked out...also, i start to go slower if i think other thoughts are intruding, cuz this ensures that im thinking abt each word and its meaning =)

Invocation against the distractions of Satan during the prayer and recitation of the Qur'an

'A'oothu billaahi minash-Shaytaanir-rajeem.

(Say:) I seek refuge in Allah from Satan the outcast

(then spit to your left).

(Do this three times reciting in Arabic.)

Trust me it's not you it's the devil whispering in your ear to make you mess up, this happened to me but it stopped. I would get so nervous and wonder did I pray the right amount or did I do something wrong. Here's this video, it's so true and maybe you'll find answers. Salaam.

first of all, have a niyyat (intention) to pray, and try to clear your mind of wordly thoughts and always try to remember that Allah is watching you. and praying is the closest you get to Allah.

i would suggest you to learn the translations of the various arabic surahs and verses that you recite in the prayers so when you are reciting them in your prayers, your mind should be concentrated on what you are saying in your prayers. your eyes should be still and not wandering here and there but if you want to help with your concentration, you can close your eyes and concentrate on the translation of the verses you are saying.

this is what i try to do to keep worldly thoughts out and be close to Allah in my prayers. helps me quite alot

Just keep ur mind in the prayer.Like when praying visualize reading something from the prayer u studied.

Find a word that has meaning for you, something that vibrates in your soul. When you notice your mind beginning to drift, softly say that word, let it remind you, gently guide you back to the quiet place of prayer and meditation. Don't force it and don't worry that your mind drifts. It happens to everyone, you just need to recognize it and bring yourself back to where you want to be.
