Counseling Statements can be issued for a number of things. Anywhere from Financial issues, to improper uniform to include hair (AR670-1), unincouraging behavior, speeding tickets, failed physical fitness test (APFT), disrespect of the officer corps. Non commissioned officers corps. Failure of the body fat %. Anything that grouds for arrest by Military police or civilian. Barracks inspection failure.
But on the flip side.. Counselings can also be issued for positive behavior. Like said list on the positive side.

It's not a legal document so the errors and the fact he was in civilian clothes doesn't mean jack squat. A counseling statement is a written record of a conversation that took place, basically, and the only thing you can do is rebut it in the space provided. There is nothing, nothing, nothing that makes a counseling statement invalid. If at the moment I was counseling you and these are the words I used orally towards you and basically the words I wrote in the counseling statement that reflect what it was we talked about, that's all a counseling statement is, a concise written record of a discussion. You can disagree with what I told you, write a rebuttal to it if you want to but that is about it. Just because I forgot to cross a T or dot and I doesn't mean the counseling record is no longer valid. She can ask a third party if she wants to, go raise all the stink you want to but the only thing she will acccomplish is making herself look like a fool. All she was told was the purpose of the counseling, key points and what they were going to do to work on to remedy what we were doing wrong in the first place to ensure it doesn't happen again. It isn't a legal document that a Judge can throw out of court because your name might be mispelled.

The most commonly thought of version of a counseling statement is indeed what has been said above.
The more appropriate use of a counseling statement is give the soldier feedback on his performance GOOD or bad on a regular basis. When properly used the Soldier is aware of what he is doing good as well as what he needs to improve and HELPS create a plan to achieve that.
It also provides a written record of these counseling sessions to document small achievements and successes as well discrepancies in the Soldiers performance (as already stated).
Unfortunately counseling statements are generally used only for negative documentation, but yes that is one part of their functionality and can be for anything from unshined boots to drunk on duty. The latter is generally a part of UCMJ/Article 15 actions as it is considered a serious offense.

A counseling Statement is also known as a DA 4856. It is pretty versatile You receive Initial counseling when you get new leadership. This explains what his standards are and what he expects from you. You receive Monthly Performance Counseling. This covers many things like, performance that month, Physical fitness level, Tarining, Upcoming events, Military Schooling, Civilian Schooling, Promotions. Then we have what is refer to as Negative Counseling witch is for any thing you could possibly do wrong.

A counseling statement can be used for negative and positive actions.
You do counseling throught the year, not just when someone does something bad.
You also counsel people and let them know they are doing a good job.
Some leaders wait until the end of the year to let someone know they are doing a bad job, when in fact they should have been telling him or her from day one what was expected of them.

A counseling statement is just that, and nothing more. It's intended to be a written record of the counseling session. It is not required that the spelling or punctuation be correct, regardless of how unprofessional it may look or seem. My recommendation is for your wife to stop getting in trouble, "man up" to whatever she got counseled for and stop trying to weasel out of it by looking for legal loopholes on a counseling form.
