I met this guy who i really like. Hes an ex heroin addict, but hes been clean and sober for over 20 years. Ive been thinking about if theres a chance he may have Aids from past dirty needles. should I worry about this after 20 years? Any advice?

Yes, it is possible that he has HIV. He might not even know if he does.
If I was in your position, I would have a discussion with him about it. If he was not willing and happy to take a blood test for your safety and his, perhaps you should not be pursuing a relationship with him.
My suggestion would be that you use all safe-sex precautions in any case.
Oh yeah - and get your own needles :-)
