Primary suspected causes would be ulcer(s) or "heartburn". Appendicitis? You'd be feeling a little more than a burning sensation. Hep-B? Nah. Not common enough. Could also be other causes too!

Heartburn is an uncomfortable feeling of burning and warmth behind the breastbone (sternum) but sometimes rising as high as the neck. Reasons: The reference to spicy food, the pain being under the breastbone, and coffee. Coffee is known to give heartburn. The most reasonable answer is heartburn, without other information. I think an ulcer is more pain from the stomach.

Both coffee and spicy foods are terrible for heartburn.

An ulcer wouldn't be behind her breast bone, STD and chest pains don't go together to my knowledge and appendicitis pain would be near the appendix.
