She probally has grown very attached to it and since it is always with her and she also might think it is a puppy or living thing. or she might think that IT is crying when you squeek it and thinks she has to protect her puppy or so.

aww how cute! it sounds like her maternal instinct is kicking in. is she spayed? if not have you considered letting her have a litter? sounds like she'd be a great mommy. if it bothers you that she is so attached, try offering a treat every time she succsesfully hands it over and is well behaved. treat the toy very nicely and let her see you pet it and love on it so she thinks it is safe with you. don't squeak it because she may think that it is crying or calling for her like a pup would, just take it one step at a time. and don't forget to offer lots of praise for her when she does what you want.

She may be having her first season. My sisters dog, Molly gets very protective of her teddy when she goes into season and won't let any one touch it, then when her season has finished she rips it to bits. Has she had a season yet, if not look out for small drops of blood and this will confirm it. All dogs react differently when in season and this is just one of the things that can happen. Don't worry about it.

She may like it and be afraid she is hurting it!
I had a dog that did that. Your dog sounds really smart:)
My dog now if given a piece of bread will carry it around and whine - probably because she doesn't like it enough to eat it and doesn't know what to do with it.

I had a poodle like that she treated it like it was her pup. She was not a young dog at the time and we had another younger dog as well who was not allowed to touch her "puppy".
