If it's just a one-time thing like Halloween, and he's dressed as "a guy in a dress", then he should use the gents' restroom.
If he's fully adopting the female lifestyle, and is gay, then he may use the ladies' restroom. However, if he is straight, then he should find a unisex or private bathroom - it's not really fair to subject other ladies to a hetero male gaze when they think they are safe in the company of other females.

If he is a transvestite, he should go into the gents. If he is a transexual, then the ladies. I was once in a West End restaurant and visited the toilet for the first time. I was a bit disconcerted when mid pee, I heard a man's voice. I had a horror that I had gone into the wrong toilet. When I emerged, I found a trans(not sure whether vestite or sexual), doing his makeup. It was kind of OK, just a bit wierd (but perhaps I had lived a bit of a sheltered life!)

He needs to use the men's room. I was with a group of friends one night and the B/F-G/F (He was in the middle of having the sex change operation) of one of the women went into the women's room and totally freaked out a woman in there. Even though he had a dress and makeup on you could tell it was a guy.

ok, this is a bit of a difficult situation. If he looks like a man in drag - he goes into the gents and if he looks like a woman in drag - he goes into the ladies. Now - reguardless of what ever type of drag he's in - why the hell do you go out with him if he looks like that?

to be honest he should go to seperate one such as disabled, (Not stating he is disabled in any way shape or form), as we used to have a transexual in work and he/she could not go into mens or wonens, so we had to have a unisex one labeled so.
I am not being cheeky with the label disabled, but it can be used for either or !!!!
