For a renal ultrasound, where I work, we don't make you put on a gown. An ultrasound is done with gel over the area of interest and the transducer (camera) on top of the skin. We cannot do ultrasound through clothing or a gown. We just have the patient remove their shirt, if they are male, or raise the shirt to just under bra level if they are a female. We also need to image your urinary bladder, so we just unbutton the pants and lower them and your underwear a little bit....I don't even see pubic hair! You can still keep your modesty for a renal ultrasound.
To image the kidneys, you are laying on your back, and possibly turned up on either side. You are not on your stomach. For the bladder, you are flat on your back.
If you have to put on a gown, you still have to raise the gown to get to the skin over your kidneys. So, if you are wearing a gown, you are actually less covered than if you are wearing your clothing. Just make sure you are wearing a two piece outfit, and not a one piece dress.

The kidneys are on the lower right and left side of the back so you'll be on your stomach anyway probably.
The gown will be a standard cloth hospital gown, it goes to your knees, has short sleeves and is open in the back but has ties to tie it shut.
If the technician needs to move the gown aside, they should cover your bottom (if you are on your stomach) with a blanket, below where they are doing the ultrasound - if they don't, then ask for one. Same thing if you are on your back. They are actually taught to do this, to maintain patient modesty.
Don't worry, they see every part of the human body dozens of times a day, and don't give it a second though. It's all just routine to them, so try not to be self conscious.

It will be the hosptial gown with nothing underneath. They will usually give you a blanket to cover with and are very good about trying to preserve your privacy. Only the technician and you will be in the room so its not like you will be on public display.

I do not like gowns. I am comfortable with medical professionals without a gown. Actually I feel more exposed with them raising the gown or reaching under it.

they don't always get you naked. my patients that are shy get to keep their underwear on. the sonographer has to be able to get to the area between your ribs and about your pants line on your back. this means that they will be smearing lube jelly on your back and as it gets warm, it gets runny. so if you chose to keep your underwear on, chances are they will get jelly on them. most sonographers are really cool about it. just ask to keep them on. and if not, ask to have a towel you can lay over your butt. they won't need to get to your front unless they decide to look for gall stones.
