Revelation (or the Apocalypse) was a type of writing common in the era it was written. It portrays the eternal fight of good and evil and shows that in the end, good will win. It's a book of hope. It's not meant to foretell the future in any specific sense. But, every generation likes to fashion it foretell some dramatic end of time battle. So far, all those predictions have been wrong, but people keep trying - like the Bible has some secret code that only a select few can understand. What rubbish. The Bible is a record of mankind's understanding of God's self-revelation.

Apocalypse means a revelation or uncovering. Apocalyptic literature became very popular in the Middle East around three centuries before Jesus was born. It was a style of writing which allowed readers to decipher the author's meaning while keeping others, often oppressive governments, in the dark. Such a literary style is not to be taken wholecloth literally. It is largely symbolic.

No. Why would it refer specifically to America?

no. It was prophesizing the fall of the Roman Empire. Since it didn't happen that way, every few months/years/decades, there a new, improved reinterpretation to fit today's "signs."
Luckily, it will be wrong, just as the thousands of other doomsday predictions were.
