bruise runs from halfway up forearm to halfway up upper arm, elbow is very red all the way around and feels very warm to touch

The bruise is normal if the person drawing the blood did it badly. However, the heat and the redness isn't. . . . sounds like an infection, maybe. I'm not a doc but I say GO IN AND SEE YOUR DOCTOR!

It looks like you have blood in the tissue because due to the needle's dislodgement in to the vein, but because it's been 5 days and you said that the bruised area is extended and your arm is painful and swolen, I think you should go to the Doctors to have it checked. Bruises are normal until certain surrounded area where the needle is inserted but in your case it extended more in diameter. Check it out with your Doctor. Good Luck

It can happen, but usually doesn't. Blood escaped from the extraction point to under your skin causing a bruise. If it si growing larger, see a doctor at once because a tear in the blood vessel may have occurred. If it's going down, it's probably healing and will eventually go away. You may still want to inform your doctor because he/she may want another blood test someday.

Warm to touch usually means infection. You might want to call the doctor. It may be cellulitis. It happens some times when you get shots or blood tests. I have diabetes and inject my self every day and it has happened to me.
