Well, there are two ways to get your "Chinese name". Through phonetics and through meaning. The phonetic seemed pretty lame to me, so what I did to find mine was to look up the meaning of my name. David means "beloved". Then to http://www.instantspeakchinese.com/dictionary/ and looked up "beloved" to find 心爱 as my "Chinese name". As I understand it, the Chinese can pick their name from the entire language, so whatever your name means should translate into a true Chinese name.

Naming in Chinese culture is a very important matter. Sometimes when people name their babies, they even hire fortune tellers to give suggestions, because we tend to think the strokes of the names are strongly associated with the fortune and fate of the baby. But of course, the decision is still on the parents.

You can find out the meaning of your original name and ask a native Chinese speaker to name you according to the meaning of your original name. I have heard of a teacher in my previous school naming a Chinese as a Second Language student out of the meaning of his original name, and of course, she made the name sound nice and perfect to native Chinese speakers.

You can also think about what meaning(s) you want your Chinese name to carry, and ask a native Chinese speaker to help naming you.

As for the surname, many Chinese language learners choose them according to the first sound or first syllable of their original surnames. For example, people surnamed Baker could adopt 裴 (pei2) as their Chinese surname; people surnamed Swift could adopt surnames 施 (shi1) or 史 (shi3).
